The MDH Hospice provides compassionate care for people and their families in the comfort of their own home, nursing home or assisted living facility. Our professional team made up of physicians, registered nurses, therapists, social workers and certified nurse aides provides care allowing quality time focused on what is most important to you.

While hospice enrollment signifies a terminal prognosis, it also provides the most unique and personalized health care support for both the patient and family currently available. Hospice care is focused on CARE and COMFORT over cure of the disease.

The MDH Hospice provides care covering all of McDonough County. A registered nurse is on call 24 hours a day, 7 days a week to answer questions over the phone or provide a professional visit. MDH Hospice provides admission visits on weekends, holidays or after hours.

Our agency is open Monday through Friday from 8:00 a.m. to 4:30 p.m. Our office is located at 503 East Grant Street, just west of the hospital main building.

MDH Hospice Earns CHAP Accreditation

Community Health Accreditation Partner, Inc., (CHAP) announced that McDonough District Hospital Hospice has both been awarded CHAP Accreditation under the CHAP Services for Standards of Excellence. By achieving CHAP Accreditation, MDH has also been deemed to meet the Medicare Conditions of Participation and is certified as a Medicare provider.

This achievement reflects the compassion, dedication, and commitment to excellence that our team demonstrates every day for our patients and their families,” said MDH Home Health and Hospice Director Michaell Zimmerman, RN. “Going through the accreditation process challenged us to look closely at our practices, strengthen our systems, and ensure that our focus remains where it belongs - on our patients and their individual needs. CHAP accreditation is an important recognition of the work our team does every day, but more importantly, it reinforces our commitment to continuous improvement and excellence in care. This accomplishment validates the quality of care we strive to provide, and it gives our patients, families and community even greater confidence in The MDH Hospice and MDH Home Health. While The MDH home care programs have always been accredited agencies, we chose CHAP because it is CMS-approved and the only accreditor undividedly focused on home and community‑based care.”

CHAP Accreditation demonstrates that MDH meets the industry’s highest nationally recognized standards. The rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources and long-term viability. Simply stated, adherence to CHAP’s standards leads to better quality  care.

The MDH Hospice is Recognized as a We Honor Veterans Partner.

The MDH Hospice program has been named a 2023 Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) Honors Elite Award recipient by HEALTHCAREfirst, a leading provider of revenue cycle management services, CAHPS and bereavement surveys, and advanced analytics for hospice organizations.

The MDH Hospice was one of just two programs in the state of Illinois to receive the CAHPS Honors Elite status. To earn that Elite distinction, a hospice program must score above the HEALTHCAREfirst National Performance Score on all 24 of the evaluated questions. In all, eight programs throughout Illinois were named Hospice CAHPS Honors. Throughout the United States there were 61 Hospice programs to earn Elite status.
