MDH Home Health Care takes care of you where you are most comfortable - your home. Our staff includes highly skilled and compassionate professionals. Home Health care is ordered by a physician and a personalized program is developed with you to meet your individual health needs promoting a full recovery and independence in your home.
Home Health skilled services are available to children or adults of any age in need of nursing or therapy services. Some of the specialized expertise of home health staff includes wound care, IV medication administration, medication teaching or care for orthopedic patients. MDH Home Health Care provides services through regularly scheduled home visits covering all of McDonough County.
Members of The MDH Home Health Care team include registered nurses, a certified wound ostomy nurse, physical, occupational and speech therapists, medical social workers and certified nurse aides.
Medicare and Medicaid cover home health services. In addition, Home Health Care is covered by most insurance plans, including HMOs and managed care organizations.
MDH Home Health Care is open Monday through Friday from 8:00 a.m. to 5:00 p.m. A registered nurse is on-call 24 hours a day, 7 days a week, available to talk over the phone or make a visit. MDH Home Health Care admits patients on weekends, holidays or after hours. The MDH Home Health Care is located at 503 East Grant Street, just west of the main hospital building.
Home Health Earns CHAP Accreditation
Community Health Accreditation Partner, Inc., (CHAP) announced that McDonough District Hospital Home Health Care has both been awarded CHAP Accreditation under the CHAP Services for Standards of Excellence. By achieving CHAP Accreditation, MDH has also been deemed to meet the Medicare Conditions of Participation and is certified as a Medicare provider.

CHAP accreditation is an important recognition of the work our team does every day, but more importantly, it reinforces our commitment to continuous improvement and excellence in care. This accomplishment validates the quality of care we strive to provide, and it gives our patients, families and community even greater confidence in The MDH Hospice and MDH Home Health. While The MDH home care programs have always been accredited agencies, we chose CHAP because it is CMS-approved and the only accreditor undividedly focused on home and community‑based care.”
