Going Home

Your Discharge

When your doctor decides you are ready to leave the hospital, a discharge order will be written. You may want to make arrangements with a family member or friend to help you when it’s time to go home.

Discharge Instructions

Your doctor and nurse will give you instructions about post-hospital care. If you have questions about your diet, activities, or other matters, please be sure to ask.

Medications

If your doctor writes a prescription for you, we would be happy to call it in to the pharmacy of your choice.

Personal Belongings

When the details of your discharge are arranged, a nurse or other staff member will assist you with gathering your possessions.

Escort

When you are ready to leave, a staff member will escort you to the front entrance and help you into your car.

After You Are Discharged

There are occasions when you may need additional treatment or care after you are discharged from MDH. A member of Advocacy Services will assist you with these special needs.

MDH Home Health Care

Home health care services are an option for you, if needed after discharge. MDH Home Health Care provides specific health care needs and must be ordered by your physician. Our Home Health Care team is comprised of registered nurses, certified nurse assistants, medical social workers, and a rehabilitation team for physical, occupational and speech therapy. Working together as a team, McDonough District Hospital wants to provide a smooth transition from hospital to home care.

The MDH Hospice

The MDH Hospice provides comfort and compassionate care for the terminally ill and offers support for families. Hospice assists with palliative care and understands your need to make the most of each day. You need not be homebound, and care is provided in your place of residence or in most area nursing homes. Our hospice team is comprised of your personal physician, hospice medical director, registered nurses, social workers, clergy, certified nurse aides, and specially trained hospice volunteers. If you or someone you know is facing a terminal illness, please contact The MDH Hospice.
