The MDH Hospice and Home Health Care offices are located in the Medical Arts Building on the MDH campus, just east of the hospital main building.
The MDH Hospice was established in 1981, providing compassionate care for people and their families at the end of life in Macomb and surrounding community. Hospice care includes physical, emotional, social, and spiritual care. Our comprehensive professional team made up of physicians, registered nurses, social workers, clergy and certified nurse aides provides care for you in the setting of your choice allowing quality time focusing on the things most important to you and time with family and friends.
While hospice enrollment signifies a terminal prognosis, it also provides the most unique and personalized health care support for both the patient and family currently available. The unique nature of hospice care for a terminally ill patient is the focus on CARE and COMFORT over cure of the disease.
The MDH Hospice provides care within a large multi-county area covering all of McDonough County and portions of Fulton, Hancock, Shuyler and Warren counties. View our service area map
. A registered nurse is on call 24 hours a day, 7 days a week to answer questions over the phone or provide a professional visit. MDH Hospice provides admission visits on week-ends, holidays or after hours.
Our agency is open Monday through Friday from 8:00 AM to 4:30 PM. Our office is located in the Medical Arts building on the MDH campus, just east of the hospital main building.
Hospice volunteers are specially trained and are available to visit, run errands or perform other tasks to assist a patient and family.
The MDH Hospice is Medicare certified, licensed by the Illinois Department of Public Health accredited by the Joint Commission and Recognized as a We Honor Veterans Partner.
MDH Hospice accepts death as a normal part of life and provides privacy, dignity and highly specialized care for the patients they serve.
