(However, you may complete the application on-line before printing and signing the application.)  Please print the form, sign the last page, and return the completed copy by US Mail (MDH, Attn: Human Resources, 525 E. Grant St., Macomb, IL 61455), FAX to (309) 836-1677, or drop it off at the Human Resources Department at MDH Monday - Friday from 7am to 4:30pm.
